On Saturday, October 26, 2019, I went to the CVS at 220 Triangle Road, Hillsborough, NJ 08844 to get a flu shot. When I returned home and reviewed the paperwork I was given after the shot, I saw that I was given another person’s paperwork. This brings up two questions: How did such a mistake get made, and where is my paperwork?

The fact that this document includes name, address, date of birth and doctor’s name is obviously quite troubling in terms of HIPAA regulations and the potential for identity theft.

What makes this incident even more troubling is the careless and unprofessional manner in which the CVS store employees handled this problem. I called the store and first spoke with Ernest Souamy, who identified himself as a store manager. When I explained the situation, he said he would need to refer this matter to the store manager. Turns out he was a store manager in training who had misrepresented himself as a store manager. Then I spoke with Chris Sundquist, an assistant store manager. While his tone and demeanor were somewhat rude, he did take steps to contact the pharmacist to look further into the matter. I was told the pharmacist would call me back either later that evening or else first thing the next morning, which was Sunday, October 27. On Sunday morning, I called the store when I hadn’t heard from anyone by around 11:35. I spoke with Chris again and asked why the pharmacist hadn’t called. He expressed surprise as he had specifically instructed her to call me as soon as possible. She finally called about 45 minutes later. The pharmacist is Asma Ansari. When I asked her why she hadn’t called me back, she said she thought it would be better to wait until after she filed her HIPAA reports. I explained that I was waiting for a call because a commitment had been made that someone would get back to me.

The staff was unsuccessful in finding my paperwork. I asked Chris to call the person whose paperwork I have in my possession, thinking that maybe our papers got switched . She claimed that she threw away the paperwork without looking at it. Either way, my information is just floating around somehwere creating a potential for identify theft. As far as I know, the store has not made any other attempts to locate my paperwork.

I have asked repeatedly to speak with Kentaro Araki, the store manager or with Abdulla Ajrula, District Leader. If nothing else, I would think that CVS management would want to officially apologize to me for this egregious and sloppy error and let me know what CVS is going to do to address the situation. As of Wednesday morning, October 30, I have not been contacted by either of these people.

The disrespect I have been shown by CVS is appalling. Beyond the actual sloppy performance of the pharmacist, the complete disregard and lack of caring shown by your staff is unbelievable. I was lied to numerous times, ranging from the store manager in training who misrepresented himself as the store manager to the commitments made for call backs that never came. Please contact me immediately so we can discuss live.

I have 2 cvs.com charges on my credit card bill. 1st on11/20 for $22.00 and 2nd one on12/13 for $9.00. I did not charge any items through the internet, so can you please clarify this? Thank you kindly for your cooperation.
